Jono took off his bag, coat and shoes and got into the tent. Jono was listing to a lot of music. Jono got out of the tent at 12:40 pm and stated making his lunch. After lunch (13:05 pm) Jono stated his first art section, painting on the large canvas with sand and flour and glue down his greeting for his Christmas card to granny. At 13:15 pm Jono finished art and sat down and watch videos and listen to music and some ripping. Jono started making his snack at 14:30 pm. After snack Jono started his learning at 15:00 pm Jono wrote “Happy Christmas” by himself. After learning (15:20 pm) Jono started his second art section, making Christmas card. | Sophia_Ramsay |
18 | 05/12/2024 11:18 AM | (03/12/24) Home handover: (Si Orphan) Jono slept through the night. Jono referenced the car when we were sat at home. We left his house at 10:35 am. Little Ilford session: Jono was a bit hyper on the walk over to the session (making loud noises, “ha”, quick breaths). On the way to the session Jono referenced “Chinese food”, “Newspapers”, “Delivery”, “Birthday" and “Pharmacy”. We arrived to Little Ilford at 11:20 am. Jono took off his bag, coat and shoes and turned on his white noise machine. Jono watched videos on his phone. He made very loud hyper sounds whilst in the tent (coughing laughs, “Ha”) and got out of the tent two times to run to the kitchen and back. At 12:36 pm Jono got out of his tent to start making his lunch. After lunch Jono did his first art session, painting with sand and flour on a large canvas. At 13:37 pm Jono went back into his tent. Jono got out of the tent to go to the toilet. Jono ran between the tent and the kitchen a few times. At 14:30 pm Jono got out of his tent to make his snack. He asked for Newspapers to rip during snack, and messaged Sophia on Whatsapp. After snack Jono did learning: He wrote two Christmas card greetings, emailed them to Sophia, played the Christmas pattern game and asked to do two worksheets (Superdrug, B and Q). After learning Jono did his second art session. Jono decorated three Christmas stocking cards. He concentrated really well during his second art session. Jono finished his second art session at 15:30 pm. Jono sat and ripped up cardboard and Newspapers, he referenced WAVE Little Ilford and Pharmacy Paper Bags. Jono went back into his tent at 16:00 pm. At 16:30 pm Jono got out of his tent to brush his teeth and go to the toilet. |

19 | 13/12/2024 12:46 PM | 09/12/24 Family handover: (Al and Si) Jono stayed one extra night at Liverpool as of the storm and went with his family to the doc to get his grandmother checked up. When there got back from Liverpool Jono went to wave and seemed stressed for about 10 min then was fine. We left home at 10:50 am to go to the centre. Little Ilford -Jonathan and Sophia: On the walk to the centre Jono referenced “newspaper”, “charity shop”, “pharmacy”, “coke cola”, “Christmas”, “birthday”. We arrived at the centre at 11:24 am. Jono took off his bag, coat and shoes and got into his tent. Jono was listing to a lot of music. Jono got out of the tent at 12:40 pm and stated making his lunch. After lunch (13:15 pm) Jono stated his first art section, painting on the large canvas with sand and flour. At 13:25 pm Jono finished art and sat down at his table to watch videos and listen to music and do some ripping. At about 14:15 I asked Jono if he wanted to make his snack and he said yes in a very hyper manner – laughing, closing his eyes tightly, gritting his teeth tightly and rocking. Jono quickly appeared to became calmer and made his snack quickly with no prompts. After eating Jono went on his phone for a bit of a break as he had had his snack early. After about 15 minutes Jono appeared keen to start learning – smiling and pointing enthusiastically at the laptop. During the online games Jono listened and responded well to instructions and while doing worksheets Jono appeared to concentrate well - immediately focusing and figuring out words independently. When Jono moved onto the 2nd art session he listened and responded well to instructions, appearing to enjoy doing the cutting and sticking and completing each card. We tried to tidy away the paper, card and glue that hadn’t been used but Jono insisted on throwing it all away. After which he ripped up and threw away 2 of the pieces of the Christmas cards and appeared to be going for the rest of the Christmas cards so I directed Jono to go to his tent. I then moved the Christmas cards to the lockable cupboard to make sure he didn’t rip up any more. Jono needed time in the tent and we had to set an alarm twice to be able to transition to getting ready to leave. |

20 | 12/12/2024 04:28 PM | Home: (Si) Jono went to the Christmas Party and enjoyed seeing everybody (11/12/24). He went to bed at 11pm, and was hyper. He had received presents from the staff team. Otherwise, he slept through the night. Little Ilford: We arrived to Little Ilford at 11:30. On the walk over Jono referenced "WAVE church", "Kebab", "Pharmacy" and "Magazines". Jono took off his bag, coat and shoes and went into the tent. The white noise machine was turned on and he put on videos on his phone. Jono was given three newspapers to rip up in his tent. At 12:36 Jono got out of his tent and made his lunch. Jono was hyper during lunch prep, and went back into the tent halfway through and lay down. For lunch Jono had beef mince hotpot and one glass of water. Whilst eating Jono referenced “superdrugs” and “magazines”. Whilst washing up a glass fell on the floor. Jono was immediately escorted out of the kitchen and the glass was picked up, and the floor was hoovered. Josie checked the bottom of Jono’s feet for glass but there was none. After lunch Jono did his first art session, painting with sand and flour on a large canvas. At 14:10 Jono went into his tent. Jono got out of his tent at 14:25 and ran back and forth in the church. Jono was directed back into the tent at 14:33. At 15:15 Jono got out of the tent to make his snack. Jono had a bowl of soup with one rice cake and one glass of water. We had to discard one rice cake as it had been in physical contact with raw meat. Jono concentrated well during learning, and did both worksheets and the number game. Jono then painted the pieces for the grotto art. Jono went back into his tent at 16:15. Lunch… Beef… mince… hotpot…pots… with… one… glass… of… water… Snack… bowl… of… soup… with… one… rice… cakes… and… one… glass… of… water… |

21 | 13/12/2024 04:41 PM | 13th December 2024 Handover (Si orphan): Si mentioned how Jono went to bed early and slept through the night. Little Ilford: We arrived by little Ilford by 11:30. On the walk Jono appeared calm and attentive, especially while signalling to cross busy roads as well as referencing “Pharmacy” “delivery” “four seasons’ Chinese” and “Hopeful futures”. At the church the white noise machine was turned on and Jono put on Christmas songs. At 12:30 Jono began ordering his lunch, a takeaway from Deliveroo although Jono was focused on ripping up papers and took longer than usual just before having lunch by 1:05PM with a can of coke, he appeared hyper running round the room. For lunch Jono had a kebab, chips, a salad and a glass of water. After lunch Jono had his first art session of the day painting flour on canvass and painting a house attentively, he cleaned up the work are used afterwards as well. Although Jono had a running nose and needed to constantly blow. At 1:50 Jono began running around the centre, preparing for the 2nd art session by drying the art pieces painted earlier and then went into his tent at 2:40. At 2:50 Jono got out of his tent to make his snack. Jono had popcorn and soup. Jono concentrated well during learning and did both worksheets and Christmas pattern games. Jono then glued pieces for the 2nd art session and went back into his tent at 3:50. Jono poured out the hand washing liquid. |

22 | 14/12/2024 03:38 PM | Wednesday 4th December Family Handover – Si: slept well – very loud and hyper when woke up, got up at 7 – bit earlier than usual. Staff Handover – Sophia: appeared relaxed happy, calm on walk to bus, bought lemon Fanta independently with cash at shop. Appeared calm on bus – watching out the window, smiling occasionally laughing only referencing occasionally. Continued to appear calm as we got off the bus and entered the Olympic park, appeared less interested in people around him than he usually is – less looking around and turning around, more looking forward and not stopping. Appeared a bit distracted when it came to ordering chips at kiosk in Food Hall – needed more prompts than usual to complete the order on the screen. Sat down at table upstairs started going on phone – needed prompting to get food out, did not seem very interested in eating, needed prompting to start eating and to drink water and lemonade. Eventually started eating, but kept needing encouragement to eat. Left ripping till end – ripped into fewer pieces than usual. Appeared calm on walk to bus – quiet, smiling, again less distracted by other people in the park – more concentrated on what was ahead. More hyper on journey back home – loud laughing and loud hyper noises and closing eyes tightly and rocking. We encouraged Jono to use his sensory toys, to rip the papers he had collected, to go on his phone and we tried to talk about the shops we were passing e.g., ‘Chinese takeaway, pharmacy’ but he continued to remain hyper for the whole journey. Jono appeared to become a bit quieter when we arrived home and put on ‘Moana’ and he was able to go on his iPad. For the last 30 min of the session Jono was quiet, watching ‘Moana’ and going on his iPad. |

24 | 14/12/2024 06:43 PM | 11th of December 2024 (Wednesday) Family handover (Si Orphan): Jono slept peacefully throughout the night without waking up once. He woke up around 7am and woke me up. Staff Handover (Isabella Leone-Holmes): Appeared calm and prepared upon arrival at his residence, having already finished packing his bag for the community session. Engaged in some brief conversation with staff before getting ready to leave. Appeared calm on the walk to the bus stop, closing all the gates on his road except for broken ones, which he bypassed when prompted by staff. At the shop, appeared focused as he selected a can of Fanta, pointing to it when asked by staff. Took the money from staff, handed it to the shopkeeper, and waited for staff to collect the change before leaving the shop. Appeared calm while boarding the bus, immediately checking the downstairs deck for newspapers. Found one in the designated box and carried it with him to the upper deck. Sat with staff and appeared engaged, referencing a few favourite topics (pharmacy, restaurant, Nando's) occasionally throughout the journey. Staff responded to his references, which seemed to keep him content. Continued to appear relaxed during the walk to Stratford Olympic Park, walking at a slow pace, making minimal noises, and referencing favourite items only occasionally. Appeared engaged upon entering the food hall but needed some assistance and additional prompts to navigate the self-service kiosk to order chips. Followed staff’s guidance to select options on the screen, completed the process, and waited patiently while staff paid. Once seated upstairs, needed prompting to take his pre-packed lunch (rice with vegetables and the can of Fanta purchased earlier) out of his bag and begin eating. Took his time eating, occasionally pausing to look around, but eventually finished both his lunch and chips without issue. Appeared calm during the walk back to the bus stop and seemed interested in taking the slightly longer route through the park at his request. Displayed focus and minimal distraction throughout the walk. Checked the downstairs deck for newspapers upon reaching the bus stop but did not find any. Remained calm as he moved to the upper deck to sit. During the return bus journey, appeared hyper, laughing loudly, making repetitive hyper noises, and rocking slightly while tightly closing his eyes. Staff encouraged him to engage with sensory toys, rip the newspaper he had collected earlier, and use his phone. Despite interventions, continued to remain hyper but occasionally referenced favorite items (pharmacy, restaurant) while looking out the window. Appeared calmer upon arriving home and was assisted by staff in selecting a movie on Disney+. Chose to watch Moana, showing interest as he settled down with staff to watch it. Appeared quiet and focused for the last 30 minutes of the session, alternating between watching the movie and using his iPad. |

Due to staff illness and exams the rota for today changed quite a few times (including this morning) which may have confused Jono and caused some anxiety. Michael is not normally on shift on a Friday. |
Jono needed some encouragement to cross from the island in the middle of the road and started walking slowly. | Jono froze in the road halfway between the island and the other side of the road. He was half-closing his eyes and smiling and not responding to staff encouragement, instruction or direction. | As Jono was not responding to either member of staff, the red man had appeared and cars were starting to move. This is putting a huge strain and emotional toll on the family. It is very difficult for them all. Housing Update Discussions around housing possibilities. JO joining the housing register was ruled out about 1 year ago due to the waiting list being at least 10 years. Discussed possibility of private landlord, risks would be JO being evicted as per the landlords discretion. There is also a framework they would have to sign up to with the council which may make it difficult to find someone willing to do this. But it remains an option. Discussed purpose homes, Alice advised this was the preferred option from her perspective due to their track record and quick turn around. Wil Lewis completed due diligence before he left and seemed happy with them. Jason shared information about another possible scheme called HOLD which would involve a part buying scheme, this hasn't been looked into yet, Jason agreed to look into it. Nina advised there is no update regarding social care purchasing the property at the moment. Alice advised Hopeful Futures are now CQC registered so are ready to provide the care component when needed, this will need to be within 12 months. We all agreed a target move in date for JO is October 2025. Care costings Alice shared HF's new, increased care costings. Nina advised that a more detailed payment schedule will need to be completed. We discussed the rationale for the care hours, particularly the wake in and sleep in staff. Discussed the rationale was due to the likelihood of JO having increased anxiety and behaviours that include destruction to property and wanting to go outside to the bins. This would not be possible to manage with one member of staff. Discussed that this arrangement should be reviewed after 6 months as a wake in and sleep in is unlikely to be required indefinitely. We also discussed the day service component of JO's support which is included in the costing breakdown and covers the renting of the hall, bills, resources for the activities etc. We also discussed the need for a new MCA and to complete DoLS application in tandem when a property is identified, this will be completed by a social worker when allocated. 10 | 03/02/2025 01:29 PM | 18/12/2024 | 09:30 PM | Wednesday 18th December 2024 Meeting with Sue Giles (Speech & Language Therapist) and Sophia Ramsay (Programme Lead for JO Programme) Sue shared that she was going to discharge Jono and would like to discuss what recommendations had worked and what had not worked. Sue also shared that if at any time we wanted to have any further help or input we just needed to get in touch with the health team. Recommendations that worked Expanding Jono’s vocabulary: Sue suggested that the learning activities should also try to expand Jono’s vocabulary, so we have been working on expanding Jono’s references and interests. For example, when Jono references Coke we sometimes also talk about Pepsi, Fanta or Sprite. Fanta, Pepsi, Sprite, Appletizer on community sessions Jono’s interests have always shaped the topics of the worksheets; as a way of expanding his vocabulary we have created worksheets with links to identified interests. For example, Jono likes the zoo and animal print paper so we created a worksheet on Zebras. Jono has demonstrated an increasing vocabulary through a variety of ways: - Regularly records lunch and snack and drink, often without much help and often attempting to figure out words before looking to staff for help. - Correctly and independently using these verbs: eat, cook, walk – using them in daily notes, worksheets, googling on his phone… - Often writing a word or most of a word (that the staff did not know he knew) independently. A current example is ‘dance’ Assessing Jono’s understanding of the written word: Sue suggested a variety of ways we could assess Jono’s comprehension. Labelling laminated photos of familiar stuff with laminated words: Jono has to correctly label items in places such as the LIBC kitchen or the garden at home. Jono appears to enjoy the interactive nature of labelling photographs and it has been a reliable way of assessing understanding of categories of words e.g., kitchen or vegetables and has been used as a basis for further worksheets. Change wording of worksheets – removing more words so that Jono has to spell them. The wording of the worksheets is regularly reviewed and changed to keep challenging Jono. Jono appears to respond positively to this challenge – demonstrating excellent focus and regularly trying to spell words independently before turning to staff for help. Ensure recipes are just written instructions - do not include any photos in the recipes. The recipes that Jono follows when cooking lunch at LIBC use words with no pictures - staff ask Jono ‘what do you have to do?’ and point at the instructions. Staff do not give prompts or directions until Jono has been given time to figure it out himself. Jono appears to respond positively to this – focusing well on the recipe and the cooking activities and often smiling and rocking while in the kitchen. Communication skills: Using What’s App to interact with family, family friends and staff Sue suggested putting more focus on communication with more of a communicative purpose. So, staff started encouraging Jono to use What’sApp every shift. Jono now regularly initiates contact with family members, staff members and family friends by sending messages. These messages are usually lists of objects Jono likes to reference e.g., takeaway, glue sticks, birthday… Jono also does not need prompting to check his messages, doing it by himself in the tent or at his table at LIBC or on the bus… Jono also responds to messages from other people, for example, he has regular message exchanges with staff members on a group chat. Emails Jono initiated use of email through demonstrating interest in Staff email and referencing names that he recognized; for example, Alice (staff member) or Carys (family member). Now Jono regularly initiates email exchanges with family members and staff members. For example, Jono has emailed Alice about having a takeaway together or having a party. Using his phone to communicate what he wants to talk about. Sue suggested encouraging Jono to use his phone to help him clearly communicate what he would like to talk about. Jono will now regularly use google to communicate to staff and family topics or objects he would like to discuss, for example, Chinese or craft. Jono has also independently discovered google maps, which he now uses as another way to communicate topics he would like to discuss. Staff ask Jono to type a word when he is struggling to get staff to understand what he is trying to communicate and often he is happy to do so. Developing relationships with others: Intensive Interaction – As a way of developing Jono’s communication Sue suggested introducing intensive interaction during sessions. Jono has responded positively to staff copying + responding to his vocalisations + hyper noises and actions such as rocking and dancing or running during LIBC sessions and in the community. Jono has appeared to seek further interaction with staff in response and appears to have become more affectionate with staff, for example, putting his hand on a staff members cheek or under a staff members chin. Jono now appears to seek more time with staff members – choosing to spend the time he would usually be in the tent sitting with staff. Jono also appears to have become more interested in people in general – when we are out in the community Jono demonstrates great interest in people walking by and those sat at other tables. This greater interest in people can be seen in Jono’s ability to communicate in more intense environments: Jono has developed his ability to manage and communicate in more intense environments. Every week he enjoys time at the Olympic Park Food Hall which is busy and loud. Jono has also done well making connections with people at the weekly church services and with contemporaries at Wave services. For example, sharing activity resources and doing activities at the same table. As a result of this development Jono has been able to go swimming for short periods of time and we are planning on going to the cinema (which Jono has requested). Recommendations that didn’t work Interactive stories – As a way to support and assess Jono’s ability to take in information and use it to make choices Sue suggested creating interactive stories about familiar topics e.g., going to Greenwich. This idea did not work though as after demonstrating the story and Jono going through it a few times he understood there were multiple options and wanted to do each version of the story every time we did the story. So, we were unable to assess r reliably engage with Jono’s ability to take in information and make choices. Taking in information – Another idea for assessing Jono’s ability to take in information was to add voice recordings on the laptop for when Jono makes lunch so Jono can get used to taking instructions straight from the laptop rather than the person standing next to him. However, Jono did not appear to respond to the instructions and appeared confused. The staff thought this might be too different a step, but that it might be an idea to come back to at a later date as Jono’s ability to take in information independently has been steadily developing; particularly in terms of cooking. Concept of time – Sue suggested creating a calendar with Jono with what he would do in a week – this will only include basic things and nothing that may be overwhelming such as a holiday – as Jono already does the date in the centre, this could help him understand the concept of time better. As this was an activity that would take time, and as Jono enjoys using his phone to google independently, he would appear to get distracted or bored and want to stop creating the calendar and go onto Google. The staff felt this could be an idea we could come back to as it could provide a sense of structure and security when Jono is feeling more anxious and overwhelmed. Mirroring words – To help Jono communicate his message and to develop his comprehension skills Sue suggested that staff write a word (the object or topic e.g. craft) on their phones when Jono references something to ensure they had understood correctly. When staff tried to put this into practice Jono appeared to get frustrated and would take their phone down and put it on a nearby surface or hold it while he referenced again. Staff wondered whether it felt to Jono that staff were not listening to him. Introducing connective words and prepositions – As a way of assessing Jono’s comprehension Sue suggested introducing changes to prompts/instructions of regular activities (such as making his snack) with words such as ‘with’ or ‘on’ or ‘under’. Staff attempted to introduce this exercise with various activities but Jono did not appear to understand. However, as Jono’s vocabulary continues to develop it is a method of assessment, I would like the staff to come back to. Recommendations that haven’t been taken up yet. Staff scrap book – To help Jono manage the fact that many of his support staff only stay for a couple of years at most Sue suggested putting together a physical or digital scrapbook of past and present support workers with their photos and names. Unfortunately, we have not managed to get round to this idea. However, it is a popular idea that we would like to put together. Jono reading instructions for craft activities – As part of assessing Jono’s comprehension skills Sue suggested introducing written instructions for a new craft activity – e.g. papercraft animal. We have not introduced this yet as we were introducing the written instructions to cooking session and we felt it would be too much change at once to introduce the change to an art session as well. However, Jono appears to have become comfortable working with the written instructions in the kitchen so the staff feel it is time to introduce the written instructions to the craft sessions. Changing small items and order of recipes - As part of assessing Jono’s comprehension skills Sue suggested regularly introducing small changes to the recipes that Jono is using, to ensure he is reading rather than remembering the recipe. As Jono appears to have become comfortable working with written instructions now would be a good time to introduce the challenge of regular changes to the recipes. Further discussion Shopping – We also discussed that as part of developing Jono’s life skills we would like to support Jono to start shopping for his meals at LIBC. Sue suggested the use of a visual shopping list and said that she would send us information about the idea. To aide communication Sue also suggested a communication app called Speech Assistant. It is an app to help people express themselves and can be customised. Further actions • Sue stated that she would follow up with the dental team at the Royal London Hospital for appointments for Jono. • Sue stated that she would send a letter to Al and Si to say that Jono had been discharged. • Sue stated that she would complete a Communication profile for Jono and send it to Al and Si and the programme. They advised that Purpose Homes is a provider of property for people with learning disabilities, autism and complex needs. They work to understand people's needs and to then provide long term, suitable housing where people stay for life because they want to and because the environment is built around them. They find an appropriate house/building and then adapt this to the person's needs. They complete an in-depth environmental assessment for the person to make sure that their environmental needs will be met. They work with housing associations who then manage the property, the tenancy and the environment of the property. They tend to work with the housing association called Inclusion. DWP pay for the rent. If the property becomes empty for whatever reason, there is a 3 month grace period where the rent is covered, following this it becomes the commissioners responsibility to pay, Philip and Michelle advised that this is low risk in their experience due to 1. No one they've housed ever deciding to move out and 2. If for whatever reason the property was vacated there being such a high demand for bespoke services such as this that someone else would be placed there easily within the 3 month grace period. Jason asked about the nominations and void agreement, purpose homes advised this would be 20 years to go alongside the lease. Jason also asked about upfront funding costs. Purpose homes advised that the property and all required adaptations are 100% funded (usually by acquired pension funds or similar) and there are no upfront or capital costs required from the local authority. If the commissioning authority pulled out for whatever reason they would then need to pay for the legal/building fees done which is usually around £7,000 - this covers the external costs to purpose homes but none of the internal costs to purpose homes. Jason asked if there were any economies of scale e.g a block of accommodation rather than a single property. Purpose homes advised that they have developed this type of service and gave an example of Southampton whereby they developed a block of 12 flats, the rent was in the region of early £300s, for a single bungalow it was in £500s and for single services was around £800s. Purpose Homes advised we would need to add at least 20% to rent costs for Newham. Alison asked about whether Purpose Homes provide environmental adaptations for noise given that JO can be noisy. Purpose Homes advised that they do and that this is within the environmental assessment completed at the beginning of the process. At this point of the meeting, Philip and Michelle from Purpose Homes left the meeting so that the group could discuss the other housing options. Other Options Jason provided an overview of the other current options for JO's housing: 1. Newham council purchasing a property - there is an amount of capital available to do this but the amount and details are not clear yet - Jason is meeting with Sam next week to explore further and to consider what would be practical for the council to do 2. Sourcing an existing council owned or housing association property - these are only 1 bed flats which would not be suitable for JO as he requires a 2 bed property due to need for 24 hour staff but Jason is enquiring as to whether a 2 bed property could be secured as he advised this would be a cost effective way of accessing a property 3. Home ownership for people with long term disabilities through HOLD - not sure if these are in Newham - Jason will find out about this next week 4. Golden Lane Housing who are associated with Royal Mencap may be able to provide a similar offer to Purpose Homes, Jason is going to explore this. Alice advised that when she looked into Golden Lane Housing it seemed as though a property would need to be in place and they would then manage the tenancy - Jason will find out about if they can also offer a property or not We had a discussion about the time pressures as we have been working on supporting Jono into independent living for a long time now and we all acknowledged our target move in date of October 2025. Alison provided further context; that the family situation continues to be unsustainable due to Simon (Jono's Dad) having been impacted by strokes and in need of support himself due to this as well as Alison's Mother being very unwell needing Alison's time and support. Alice advised there is no contingency plan in place should home life breakdown due to continued strain of caring responsibilities on the family. Jason agreed that he can develop the business case within the next 6 weeks and that it will be completed by our next meeting on 8th May 2025. Supported Living Budget Nina advised that we may need to split the costings of the day provision component of the overall budget for Jono's support. Alice advised difficulties with this due to the proposed supported living provision being across 24 hours, the support not being separate and it being a completely bespoke service to JO. Nina advised she will find out if we need to split the costing proposal or if we can keep it as one proposal. We arranged our next meeting for Thursday 8th May 2pm - 3pm. Actions -Jason to complete the business case by our next meeting on 8th May 2025 -Jason to find out if JO has been allocated a social worker -Nina to find out if Hopeful Futures needs to split the costing proposal and to then check with social care that the proposed hours are correct and right for Jono's assessed needs |
